What to Bring to Registration
· Parent or guardian must accompany students to registration to sign required forms.
· Child’s social security number
· Certified copy of birth certificate (Kindergarten and New Students)
· Required health forms (see below)
Health Requirements
· Pre-K: lead screening verification, completed physical, up-to-date immunization record
· Kindergarten: lead screening verification, completed physical, up-to-date immunization record, vision exam, dental exam
· 2nd Grade: dental exam
· 6th Grade: completed physical, up-to-date immunization record, dental exam
· 9th Grade: completed physical, up-to-date immunization record
· 12th Grade: up-to-date immunization record
· New students from out of state: completed physical, up-to-date immunization record and vision screening (before registering for school without exception.
Transportation
All students are required to complete the online Transportation Request Form. A student must live at least one and one-half miles from the attending school in order to be eligible to ride a bus.
