VEHICLES:UNIT 1 – 2012 Chevrolet Truck

DRIVER: UNIT 1 – Michael Lewis, M, Age 37, Yale, IL

PASSENGERS: None

CHARGES: UNIT 1 – Pending Investigation

TRANSPORTS: UNIT 1 – Driver was transported to Deaconess Midtown Hospital in Evansville, IN via air ambulance.

PRELIMINARY:  Preliminary investigative details indicate Unit 1 drove off the roadway to the left, struck an embankment and came to rest in the left ditch.
